Appointment Scheduling

Contract For Services At A Price Offered.
A confirmed appointment is considered a contract for services. Therefore, Failure to keep an appointment is a breach of this simple contract and incurs a charge.

New Clients
All new clients are REQUIRED to
prepay by providing credit – debit
card information to guarantee an
initial appointment. You will not be charged until your appointment is confirmed, unless you are a no show or late cancellation.
Once you are an established (returning) client, you can make appointments online. Ncharges are made until your appointment time, unless you are a No Show or Late Cancel.
Late Cancel- No Show Policy
Once you have scheduled an appointment, if you need to cancel or reschedule, 24 hours prior notice is required to avoid a charge.
Late Cancel – No Show Fees:
The charge for a No Show or late cancellation is 50% of the regular (undiscounted) fee for theme booked.
Sixty (60) minute Appt: $37.50
Ninety (90) minute Appt: $55.00
Payment Policies
Payment is expected at time of service, typically at the end of your session. No exceptions.

New clients are required to prepay with debit credit card or Venmo, PayPal Established, returning or regular clients may choose an option above.
If you incur a Late Cancel or No Show Fee, your account will be charged using your card on file. Paying by debit-credit card authorizes the practice to make your card the default payment for such charges.
Insurance Coverage:
It’s Complicated
In New York State, currently the only way to get insurance coverage for Massage or Manual Therapy, is under the state’s No Fault Auto Insurance law.This requires a doctor’s written prescription (state Rx form) for Medical Massage as medically necessary part of treatment plan. However, coverage is not guaranteed and is out of the practice’s control.
Insurance Assignment:
If you request that the practice agree to Insurance Assignment (the insurer pays the practice directly, you have no out of-paocket cost) the required and appropriate information will be gathered, forms completed and submitted by the practice at no cost.
Acceptance of Insurance Assignment is dependent on your carrier. Some carriers are very difficult and expensive to work with and do not pay timely. The practice reserves the right to decline Insurance Assignment.
Privacy Policy
As a healthcare provider, Integrated Manual Therapy and Wellness is bound by the same privacy rules as your doctor, counselor or attorney, under the Federal Health Information Privacy and Portability Act (HIPPA) regulations.
Your email address and phone number are collected for communication purposes only. including appointment confirmation, reminders, newsletters, announcements and special offers.
- Your contact information will never be sold to third parties,
- All personal information, health and treatment information is encrypted and stored as required, in digital format behind multiple firewalls.
- All session and SOAP Notes are transcribed and stored digitally securely off-site behind multi[e firewalls.
- Only you and your Therapist can access this information.
- Changes to these policies will be sent to you electronically (via email or text message) beofore they go into effect.

sold, you will be notified and
have the opportunity to opt out
of the transfer of Customer Lists
and information to new owner.
